This study seeks to document the prevention or reduction of migraine pain and impact on quality of life through patented nutritional supplementation and trigger management. The patented nutritional supplement is called Seratonin^TM. and has been issued a U.S. Patent specifically for migraine prevention.

This migraine prevention program has been used successfully for over 8 years. Seratonin^TM was formulated and patented for preventing migraines.

The purpose of the AMPS study is to provide evidence-based research as to the effectiveness of Seratonin^TM and the trigger management protocol. A 2002 medical school study found that subjects missed fewer days of work and school, experienced less functional impairment, missed less household work and fewer social activities, noted a reduction in overall days with migraine, as well as a decrease in the severity of their headache pain. Further research, as sought by this study, was recommended.

Documentation is an important part of AMPS to build evidence-based research. We are following the study protocol approved by the Mid-Atlantic Institutional Review Board.

• Informed Consent - to signed by you and a witness to your signature

ENROLLMENT FEES: NUTRITIONAL SUPPLEMENTS
Enrollment fees are an important aspect of AMPS. Prevention involves your commitment to learn and use what you learn. Your financial participation help allows us to enroll more participants. This is a consumer driven study. The first month you will need to subscribe to a monthly order of Seratonin^TM. Your monthly subscription rate is $49.95 per month. The enrollment kit, containing the first month supply of Seratonin^TM as well as all other supplementation, costs $97.90. Each monthly order after that will cost $49.95 (+$7.95 shiping).

PARTICIPANT COMPENSATION
You will receive a credit card refund for the value of a bottle of Seratonin^TM when we receive your study documentation. This is to recognize your time and commitment to migraine to prevention research.

STUDY PROTOCOL
During the course of the study, the patient will adhere to the following protocol:

1. Supplement with Seratonin^TM daily as recommended.
2. Use Purifica (detoxification formula) for two weeks as directed in second month.
3. Submit study documentation to DHR.
